Aspirin inhibits platelet cyclooxygenase by irreversible acetylation, thereby preventing the formation of thromboxane A2 which is a powerful stimulant of platelet aggregation. Clopidogrel, a thienopyridine, acts by inhibiting adenosine receptors, which inhibits the early step of platelet activation.
Aspirin is rapidly absorbed in the upper gastrointestinal (GI) tract and results in a measurable inhibition of platelet function within 60 minutes. This antiplatelet effect is associated with prolongation of the bleeding time and inhibition of TXA2-dependent platelet aggregation.
In the liver, approximately 15% of absorbed clopidogrel is metabolized by the cytochrome P450 (CYP) system to generate its active metabolite via a 2-step bioactivation process, whereas the rest 85% is hydrolyzed by carboxylesterase 1 (CES1) to an inactive carboxylic acid derivative.
Most side effects do not require any medical attention and disappear as your body adjusts to the medicine. Consult your doctor if they persist or if you’re worried about them
