Mechanism Of Action

Similar to other benzodiazepines, clobazam binds to the interface of the α and γ2-subunit of the GABA-A receptor. However, it is considered a partial agonist to GABA-A receptors which sets clobazam apart from 1,4-benzodiazepines which are full agonist.


Pharmacokinets

Clobazam is known to inhibit CYP2D6, affecting the pharmacokinetics of drugs such as dextromethorphan, which undergoes an 95% increase in its AUC in the presence of clobazam  Clobazam and its metabolites are excreted in the urine and feces  with the rate of elimination of clobazam increasing with age

Contra Indications

Cloam 0.5mg Tablet may cause excessive drowsiness with alcohol.

Cloam 0.5mg Tablet is probably unsafe to use during breastfeeding. Limited human data suggests that the drug may pass into the breastmilk and harm the baby.

Short-term use is not expected to cause any adverse effects in the breastfed baby, especially if the baby is older than 2 months. During long-term use, monitor for possible sleepiness.

Adjunctive therapy for seizures seen with Dravet syndrome.

Adjunctive treatment for refractory status epilepticus.

Adjunctive therapy of refractory focal epilepsy.

Catamenial epilepsy (off-label)

Interactions

orlistat, sodium oxybate. Other medications can affect the removal of clobazam from your body, which may affect how clobazam works. Examples include fluconazole, fluvoxamine, among others.
