Mechanism Of Action

Clofin GT Ointment is a combination of four medicines: Clobetasol, Terbinafine, Gentamicin, and Zinc Sulphate Monohydrate which treat skin infections. Clobetasol is a steroid medicine. It blocks the production of certain chemical messengers (prostaglandins) that make the skin red, swollen and itchy.

Pharmacokinets

Clobetasol propionate is a corticosteroid used to treat corticosteroid-responsive dermatoses and plaque psoriasis. Clobetasol propionate is a prednisolone derivative with higher specificity for glucocorticoid receptors than mineralocorticoid receptors. Following oral administration, terbinafine is well absorbed ( > 70%) and the bioavailability of Lamisil Tablets as a result of first-pass metabolism is approximately 40%. Peak plasma concentrations of 1 μg/mL appear within 2 hours after a single 250 mg dose; the AUC is approximately 4.56 μg&bu;ll;h/mL.The dosage of gentamicin necessary to produce mean peak serum concentrations between 4 mug/ml and 6 mug/ml was 60 mg/m2 per dose for all age groups or 2.5 mg/kg per dose for ages one-half to five years, 2.0 mg/kg per dose for ages five to 10 years, and 1.5 mg/kg per dose for ages greater than 10 years.Zinc facilitates wound healing, helps maintain normal growth rates, normal skin hydration and the senses of taste and smell. Zinc inhibits cAMP-induced, chloride-dependent fluid secretion by inhibiting basolateral potassium (K) channels, in in-vitro studies with rat ileum.

Terbinafine is an antifungal which stops the growth of fungi by preventing them from forming their own protective covering. Gentamicin is an antibiotic which kills bacteria by preventing the synthesis of essential proteins required by bacteria to carry out vital functions. Zinc Sulphate Monohydrate has astringent and antiseptic action.

Interactions

Clobetasol + Terbinafine + Gentamicin + Zinc Sulphate may have interaction with pain killers (aspirin), antibiotics (ampicillin, ceftriaxone, clindamycin, and vancomycin), diuretic medication (furosemide) and a local anaesthetic (lidocaine).
