Mechanism Of Action

The mode of action of aceclofenac is largely based on the inhibition to prostaglandin synthesis. Aceclofenac is a potent inhibitor of the enzyme cyclo-oxygenase, which is involved in the production of prostaglandins. After oral administration, aceclofenac is rapidly and completely absorbed as unchanged drug.

Paracetamol has a central analgesic effect that is mediated through activation of descending serotonergic pathways. Debate exists about its primary site of action, which may be inhibition of prostaglandin (PG) synthesis or through an active metabolite influencing cannabinoid receptors.

Serratiopeptidase acts as an anti-inflammatory agent by regulating inflammatory cytokines and hence the onset of chronic inflammation. It significantly modifies cell adhesion molecules that guide inflammatory cells to the sites of inflammation.

Paracetamol is extensively metabolised (predominantly in the liver), the major metabolites being the sulphate and glucuronide conjugates.

Serratiopeptidase is a proteolytic enzyme prescribed in various specialities like surgery, orthopaedics, otorhinolaryngology, gynaecology and dentistry for its anti-inflammatory, anti-edemic and analgesic effects.

Indications

Aceclofenac film-coated tablets are indicated for the relief of pain and inflammation in osteoarthritis, rheumatoid arthritis and ankylosing spondylitis. Aceclofenac film-coated tablets are supplied for oral administration and should be swallowed whole with a sufficient quantity of liquid.

Acetaminophen or paracetamol helps to reduce fever and to relieve a headache with mild to moderate pain. It is also used with other pain medications to treat low back pain, osteoarthritis, cold, and toothache.

Serratiopeptidase is often used in oral surgery for its anti-inflammatory purpose after impaction surgery, maxillofacial trauma and infections but its use should be limited in cases of abscess due to its fibrinolytic activity.
