Valsartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues, such as vascular smooth muscle and the adrenal gland.
Hydrochlorothiazide inhibits sodium chloride transport in the distal convoluted tubule. More sodium is then excreted in the kidney with accompanying fluid. Pharmacological effects begin in about 2 hours after an oral dose, peak in 4 hours, and lasts for about 6 to 12 hours
Valsartan is an orally active, potent, and specific angiotensin II type 1 (AT 1) receptor blocker (3). The AT 1 receptor mediates all known cardiovascular actions of angiotensin II, including vasoconstriction, aldosterone secretion, and sympathetic stimulation.
Absorption of hydrochlorothiazide is reduced in patients with congestive heart failure. Peak plasma concentrations are observed within 1 to 5 hours of dosing, and range from 70 to 490 ng/mL following oral doses of 12.5 to 100 mg.
