Mechanism Of Action

Serratiopeptidase acts as an anti-inflammatory agent by regulating inflammatory cytokines and hence the onset of chronic inflammation. It significantly modifies cell adhesion molecules that guide inflammatory cells to the sites of inflammation.

diclofenac exerts its action via inhibition of prostaglandin synthesis by inhibiting cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) with relative equipotency.

Serratiopeptidase reduces pain and swelling without inhibiting prostaglandins and has no gastrointestinal adverse effects. It also acts by enhancing blood circulation due to proteolytic effect, removing damaged and denatured proteins and cellular debris, and modulating inflammatory cytokines.

Diclofenac binds extensively to plasma albumin. The area under the plasma concentration-time curve (AUC) of diclofenac is proportional to the dose for oral doses between 25 to 150 mg. Substantial concentrations of drug are attained in synovial fluid, which is the proposed site of action for NSAIDs.

Indications

Serratiopeptidase is often used in oral surgery for its anti-inflammatory purpose after impaction surgery, maxillofacial trauma and infections but its use should be limited in cases of abscess due to its fibrinolytic activity

Diclofenac is indicated for treatment of primary dysmenorrhea, for relief of mild to moderate pain, for relief of the signs and symptoms of osteoarthritis, for relief of the signs and symptoms of rheumatoid arthritis, for relief of symptoms of ankylosing spondylitis and for relief of acute migraine symptoms.
