Basic Information

Mechanism Of Action

Chlorzoxazone is a muscle relaxant. It acts by blocking nerve impulses or pain sensations that are sent to brain. Typically, it is used together with rest and physical therapy to treat skeletal muscle conditions such as pain or injury.

As with all NSAIDs, diclofenac exerts its action via inhibition of prostaglandin synthesis by inhibiting cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) with relative equipotency

Paracetamol has a central analgesic effect that is mediated through activation of descending serotonergic pathways. Debate exists about its primary site of action, which may be inhibition of prostaglandin (PG) synthesis or through an active metabolite influencing cannabinoid receptors

Pharmacokinets

Analysis of plasma samples indicated a rapid absorption and rapid elimination of chlorzoxazone. Average values of the elimination half-life and plasma clearance were 1.12 +/- 0.48 hr and 148.0 +/- 39.9 ml/min, respectively.

Diclofenac binds extensively to plasma albumin. The area under the plasma concentration-time curve (AUC) of diclofenac is proportional to the dose for oral doses between 25 to 150 mg. Substantial concentrations of drug are attained in synovial fluid, which is the proposed site of action for NSAIDs

It distributes rapidly and evenly throughout most tissues and fluids and has a volume of distribution of approximately 0.9L/kg. 10 to 20% of the drug is bound to red blood cells. Paracetamol is extensively metabolised (predominantly in the liver), the major metabolites being the sulphate and glucuronide conjugates.

Indications

Chlorzoxazone is indicated as an adjunct to rest, physical therapy, and other measures for the relief of discomfort associated with acute, painful musculoskeletal conditions. The mode of action of this drug has not been clearly identified, but may be related to its sedative properties.

Diclofenac is indicated for treatment of primary dysmenorrhea, for relief of mild to moderate pain, for relief of the signs and symptoms of osteoarthritis, for relief of the signs and symptoms of rheumatoid arthritis, for relief of symptoms of ankylosing spondylitis and for relief of acute migraine symptoms.

Acetaminophen or paracetamol helps to reduce fever and to relieve a headache with mild to moderate pain. It is also used with other pain medications to treat low back pain, osteoarthritis, cold, and toothache

Interactions

Buprenorphine Chlorzoxazone may increase the central nervous system depressant (CNS depressant) activities of Buprenorphine.

Bupropion The metabolism of Chlorzoxazone can be decreased when combined with Bupropion

Despite its frequent coadministration with other drugs and widespread use, very few clinically significant drug interactions have been documented with paracetamol. Drugs that alter gastric emptying can alter paracetamol pharmacokinetics, but this does not result in toxicity in therapeutic use.
