Olumiant 4mg Tablet is a Janus kinase inhibitor used to treat moderate to severe rheumatoid arthritis. It helps decrease pain, tenderness, and swelling in the joints by reducing inflammation. It also helps slow the progression of bone and joint damage.
Olumiant 4mg Tablet can be taken with or without food, but it is best to take them at the same time each day. Your dose will depend on your condition and what other medicines you take. You should use it as your doctor tells you to. Do not stop taking this medicine unless your doctor tells you to because your condition could worsen.
The most common side effects of Olumiant 4mg Tablet include throat and nose infections, nausea, and viral infection. You may also get cold sores, a feeling of sickness, or have symptoms of a cold like stuffy nose, sneezing, and sore throat. There are other less common side effects, some serious, including blood clots and tears in the stomach or intestine wall. These need urgent medical attention. If you have concerns regarding side effects, talk to your doctor.
You may need blood tests before you are given this medicine, and you should not start taking it if you have any infection. Make sure you talk to your doctor before starting treatment if you have or have ever had tuberculosis, shingles, kidney or liver disease, hepatitis B or C, or blood clots in your legs or lungs.
Olumiant 4mg Tablet can make you more likely to get infections or worsen any current infections, so avoid contact with people who have things you might catch (e.g., chickenpox, measles, or flu). This medicine is not suitable for children under 18 years old, and you should not use it if you’re pregnant or breastfeeding unless your doctor feels the benefit is greater than the risk. Your doctor may need to examine you regularly.
Olumiant 4mg Tablet is a Janus kinase (JAK) inhibitor that inhibits the activity of selective JAK1 and JAK2 enzymes, which interfere with the JAK-STAT signaling pathway (a chain of protein-cell interactions, involved in processes such as immunity, cell division, cell death, and tumor formation).
Baricitinib demonstrates dose-linear and time-invariant pharmacokinetics (PK), with low oral-dose clearance (17 L/h) and minimal systemic accumulation observed following repeat dosing.

Baricitinib is contraindicated in patients with chronic kidney disease with an eGFR <30 mL/min/1.73 m². However, for COVID-19 patients, baricitinib is not recommended for patients with end-stage renal disease with an eGFR <15 mL/min/1.73 m².
Olumiant 4mg Tablet is unsafe to use during pregnancy as there is definite evidence of risk to the developing baby. However, the doctor may rarely prescribe it in some life-threatening situations if the benefits are more than the potential risks. Please consult your doctor.
Consuming alcohol with Olumiant 4mg Tablet does not cause any harmful side effects.
Olumiant 4mg Tablet is probably unsafe to use during breastfeeding. Limited human data suggests that the drug may pass into the breastmilk and harm the baby.
baricitinib is indicated for the treatment of adult patients with moderately to severely active rheumatoid arthritis who have had an inadequate response to one or more TNF blockers. Baricitinib may be used as monotherapy or in combination with methotrexate 9,11 or other DMARDs.
Drug-Drug Interactions: BARICITINIB may interact with painkillers (tramadol), biological therapy (abatacept), steroid (prednisolone), anti-rheumatic drug (methotrexate).
Drug-Food Interactions: BARICITINIB may interact with ethanol.
Drug-Disease Interactions: BARICITINIB may interact with pregnancy, thromboembolism, abdominal disorders such as diverticulitis, GI perforation; cancer, liver disease, chronic infections such as tuberculosis, hepatitis B and C and shingles, and any anaemias.

Your doctor may get regular blood tests done to monitor your liver function, kidney function, blood counts or cholesterol level.

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Olumiant 4mg Tablet may be taken with or without food, but it is better to take it at a fixed time.
Q. Can I take Olumiant 4mg Tablet if I have liver problems?
Inform your doctor if you have liver problems, before starting Olumiant 4mg Tablet. This is done because in some cases Olumiant 4mg Tablet may cause an increase in liver enzymes. Therefore, your liver function needs to be investigated before starting Olumiant 4mg Tablet. Although, Olumiant 4mg Tablet can be given to patients with mild to moderate liver function impairment, but should not be prescribed to patients with severe liver impairment.
Q. Can Olumiant 4mg Tablet cause cancer?
Olumiant 4mg Tablet may increase the chances of developing the risk of certain cancers (lymphoma and other cancers including skin cancers) by affecting the working of the immune system. Hence, inform your doctor if you ever had or have any type of cancer.
Q. Do I need to get any investigations done before starting Olumiant 4mg Tablet?
You may need to get your blood tests done before you start Olumiant 4mg Tablet or while you are taking it. Blood tests are done in order to check if you have a low red blood cell count (anemia), low white blood cell count (neutropenia or lymphopenia), high blood fat (cholesterol) or high levels of liver enzymes. It also helps to ensure that treatment with Olumiant 4mg Tablet is not causing problems. In addition to this, your doctor may advise you to get tested for tuberculosis before starting Olumiant 4mg Tablet, as using Olumiant 4mg Tablet may make an existing infection worse.
