Your doctor will tell you how often you need to use your inhaler. The effect of this medicine may be noticeable after a few days but will only reach its maximum after a few weeks. This medicine must be used regularly to be effective, so go on taking it even if you don't have any symptoms. That means it’s doing its job. If you stop taking it your COPD may get worse. This medicine should not be used to relieve sudden shortness of breath. If sudden shortness of breath occurs, use your rescue inhaler. To get the benefit from this medicine you need to make sure you get your inhaler technique right, otherwise, it Does not work as well.

The most common side effects are nausea, vomiting, dryness in the mouth, fungal infection of mouth, respiratory tract infection, bronchitis (inflammation of the airways), cough, sore throat, hoarseness of voice, and headache. If you get there, don’t stop taking it but do talk to your doctor. You can prevent some of these symptoms by rinsing your mouth and throat with water or brushing your teeth after using your inhaler. There are other, rarer side effects which can be serious. Talk to your doctor if you're worried about them.

Trimium Transhaler is a combination of three medicines: Ciclesonide, formoterol and tiotropium. Ciclesonide is a corticosteroid. It acts by stopping the release of certain natural substances in body that are responsible for inflammation of the airways. Formoterol is a bronchodilator, while tiotropium is an anticholinergic. They work by relaxing the muscles in the airways and widen the airways. Together, they make breathing easier.


Pharmacokinets

Ciclesonide is a novel inhaled corticosteroid that possesses a unique pharmacokinetic and pharmacodynamic profile. Ciclesonide is inactive itself and converted to its pharmacologically active metabolite, desisobutyryl-ciclesonide, in the target organ, the lungs.Tiotropium is a long-acting, antimuscarinic bronchodilator used in the management of chronic obstructive pulmonary disease (COPD) and asthma. Tiotropium acts mainly on M3 muscarinic receptors located in the airways to produce smooth muscle relaxation and bronchodilation.Formoterol, a long-acting beta 2-selective adrenoceptor agonist, produces dose-proportional bronchodilation in patients with obstructive airways disease with a reversible component. A significant effect occurs within minutes of inhalation of a therapeutic formoterol dose and persists for approximately 12 hours.

CICLESONIDE+FORMOTEROL+TIOTROPIUM may interact with bronchodilators (albuterol), steroids (prednisone), diuretics (furosemide, amiloride), pain killers (aspirin), antibiotics (azithromycin), antidepressants (duloxetine), heart-related medicines (metoprolol), antifungals (itraconazole, voriconazole, posaconazole, ketoconazole), HIV/AIDS drugs (ritonavir), mono-amino oxidase inhibitors (phenelzine),

Typical Dosage

Shake the inhaler. While you are breathing in from mouth, press down on the inhaler one time to release the medication and hold your breath for 10 seconds. Repeat until you have inhaled the number of puffs as suggested by the doctor.Afterwards, rinse your mouth thoroughly with water and spit it out.

Effects Of Missed Dosage

If you miss a dose of Trimium Transhaler, use it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

Trimium Transhaler helps relieve symptoms of severe chronic obstructive pulmonary disease (COPD) such as cough, wheez and breathlessness.

This medicine is for inhalation only. The tablet should not be swallowed.

Take it at the same time each day.

It does not work right away and should not be used to relieve sudden breathing problems. Use your rescue inhaler to control sudden difficulty in breathing.

Dry mouth may occur as a side effect. Frequent mouth rinses, good oral hygiene, increased water intake and sugarless candy may help.

Gargle with warm water after each inhalation to avoid any fungal infections in your mouth and throat.

Take the first dose of this medicine under medical supervision as it may cause wheezing or tightening of the airways (bronchospasm) immediately after using.

Very little amount of Trimium Transhaler may get absorbed into the bloodstream after inhalation. Hence, serious side effects are unlikely.

Do not stop taking it suddenly even if you don’t currently have any symptoms as that may cause your breathing problems to come back.
